I acknowledge that my child(ren) are physically able to participate in the Conqueror Sports League, and that any restrictions in activity will be submitted to the league staff prior to participation. I understand the reasonable risk involved with the participation of my child(ren). I hereby give permission for the league staff to seek/provide appropriate medical attention in the event of an accident, injury, or illness. I will be responsible for all costs of medical attention and treatment.

I hereby give permission for my child to be photographed during the Conqueror Sports League. I understand the photos will be used to keep a journal of activities, to share during power point presentations and/or reports to our donors and for promotional purposes including flyers, brochures, newspaper and on the internet. I understand that although my child’s photograph may be used for advertising, his or her identity will not be disclosed, I do not expect compensation and that all photos are the property of Conqueror Sports League.
